Intake Specialist

Purpose of Position:

Responsible for engaging new members in admissions process for referrals to S.T.A.R. - Stand Together And Recover Centers, Inc. for Outpatient Treatment services. Provides professional, warm, and inviting engagement during outreach to schedule intake appointments for new members. Ensures enrollment paperwork is complete, accurate, and maintains high payer compliance standards. Helps new members develop Individualized Service Plans (ISP) that are appropriate and meet the needs to the member. Conducts ongoing outreach to members with no contact or haven’t engaged in services for more that 10 days. Utilizes standard office software to track referral process and coordinate next steps.

Essential Functions:

· Outreach and document successful and unsuccessful attempts in the Electronic Health Record EHR for new referrals within 24 hours

· Outreach and document ongoing attempts for members on “no attendance” reports

· Consistently provides excellent customer service in all interactions with community and potential new members

· Educates potential members on what services are available

· Schedules intake and orientation appointments for new members

· Provides friendly and clear explanation of S.T.A.R. enrollment paperwork and consistently completes it with members in its entirety

· Completes high-quality enrollment paperwork; ensuring that payer compliance guidelines are met while developing rapport and engaging members to support their participation

· Works with the center receptionist and site management to communicate new member start dates and facilitate a smooth transition into services

· Establishes and maintains contact with relevant clinical team members from outside agencies during the admissions process to include Case Manager(s), Clinic, PCP, family, or other care team members as needed

· Facilitates the development of a S.T.A.R. Individualized Service Plan (ISP)

· Participates in required Clinical Oversights facilitated by a Behavioral Health Professional (BHP)

· Utilize Outlook for emails, calendar, and Microsoft teams communication as directed

· Attend all staff meetings

· Other duties as required
