Key Responsibilities:
Perform in-home skilled nursing visits, assessments, and evaluations for patients.

Document patient care accurately and timely in accordance with agency policies and state/federal regulations.
Monitor patient conditions, communicate changes to the DON and physician, and recommend interventions as needed.
Educate patients and caregivers on disease management, medications, and home care procedures.
Ensure patient safety and adherence to infection control standards during home visits.
Collaborate with other healthcare professionals to ensure continuity and quality of care.
Qualifications:
Current Registered Nurse (RN) license in the state of practice.
Minimum of one year of nursing experience, preferably in home health or a community-based setting.
Strong clinical assessment, documentation, and organizational skills.
Ability to work independently and manage a flexible, part-time schedule.
Valid driver’s license and reliable transportation.
