Job Summary
The Rehabilitation Nurse takes on some of the responsibilities of the
department, as may be delegated. He/she assists and supports the translation of
philosophy of the center into nursing practice by practicing in the planning,
and evaluation of the nursing care delivery system.

Job Qualifications
The Rehabilitation Nurse must be a graduate of an accredited School of Nursing with current registered nurse licensure by the State Board of Nursing or a Lisence practical Nurse. A minimum of three years full-time or equivalent clinical experience plus two years clinical experience in Rehabilitation Nursing preferred
Job Duties & Responsibilities
1) Assists and participates in the development, organization and implementation of the philosophy, objectives, policies, procedures and standards of nursing care in accordance with the goals of the center.
2) Confers with the Director of Nursing in assessing the quality of nursing care being delivered and recognizes the need for improving or changing nursing practices.
3) Assists with the development, direction, paricipation and evaluation of the restorative program.
4) Makes daily rounds to evaluate resident care, the progress of individual residents, monitors restorative practices, and assists staff with restorative nursing care and educational needs.
5) Remains cognizant of legal aspects of nursing practice, as wellas government regulations; demonstrates this in teaching, supervision, and evaluation of residnet care.
6) Performs fall assessments upon all new admissions, quarterly, and as needed, for existing residents throughout the facility.
7) Performs restraint assessmants upon all new admissions, quarterly, and as needed, for existing residents throughout the facility.
8) Performs rehabilitation assessmants upon all new admissions, quarterly, and as needed, for existing residents throughout the facility.
9) Is responsible for the completion of Section G on the MDS, and accompanied CAA’s and care plans.
10) Is responsible for ensuring that all responsible staff are educated, and completing restorative nursing documantation in the EHR, to include but not limied to:
a. ADL daily documentation
b. ADL (7) day look back tracking forms
c. Restorative nursing program monthly calendar
d. Restorative nursing program minutes form
e. Restorative nursing programs
f. Monthly restorative progress notes
g. Quarterly restorative progress notes
h. Endurance assessmants for any resident with more than two restorative programs
11) Is responsible for coordinating all restorative programming and documentation
12) Performs or supervises the performance of restorative care.
13) Assess all residents for
a. Need for wheelchairs or other assistive devices
b. Appropriate fit of wheelchair and other assistive devices
14) Provide for appropriate positioning devices to ensure that residents comfort, position, and safety needs are met.
15) Provide safety/ personal alarms for residents per protocol, and ensure that:
a. Alarms are in woking order
b. Restorative aides check alarms every AM and replace as necessary
c. A list is maintatined of all residents with use of alarms
16) Maintatin a list of all high fall risk residents and provide to Interdisciplinary Team.
17) Monitor dining rooms to ensure that any residents with feeding difficulties are supplied with proper assistive devices and are placed in appropriate feeding programs or referred to speech therapy.
18) Schedule restorative aides to assist with dining room monitoring and feeding.
19) Serves as liaison with all therapists: physical, occupational and speech.
20) Assures that the residents individulaized care plan is followed.
21) Participates on the Interdisciplinary team
22) Attends care plan meetings;
23) Attends Medicare meetings
24) Attends safety risk meetings
25) Acts as on-call nursing clinical resource person, as scheduled;
26) Serve in the role of Medicare Services Coordinator as needed;
27) Puts Customer Service First: Ensures that customers and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals' needs and rights;
28) Participate on the Quality Assurance committee and attend quarterly meeting.
29) Participates in the employee orientation process as it relates to his/her department.
30) Ensures that all C.N.A.’s have signed documentation of return demonstrations of all aspects of restorative programs.
31) Participate in floor staffing rotation.
32) Participate in survey preparedness.
33) Participate in weekend on call nursing rotation.
34) Participate in administrative weekend rotation.
35) Performs other duties as requested
