Overview
We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will possess a strong background in behavioral health and social work, with experience in crisis intervention and management. As a Case Manager, you will play a crucial role in supporting individuals and families through challenging situations, providing guidance, resources, and advocacy to help them achieve their goals.
Responsibilities
- Assesses and determines the service needs of the caseload to provide proper case management services.
· Links clients with other agencies, health service provider services, and support systems that assist in the maintenance and quality of life for the clients.
· Identify new, untapped community resources to increase service utilization and to meet the diverse needs of residents.
· Assists in maintaining and supporting the social, medical, spiritual, psychological well-being of all clients.
· Timely documentation of all client contacts and including contact with our service providers
· Complete client centered service plan as needed.
· Provides individual counseling and to inspire the clients to participate Recreational activities and in other service that will meet the client’s need
· Conduct bi-weekly room inspections and visits to local agencies to present the services available at Ryer and/or to conduct presentation at Ryer Avenue HDFC
-Responsible for the overall development and implementation of the recreation program.
-Plans monthly calendar of events and promotes events to the clients.
-Monitors and secures activity spaces.
-Create a summary of every event for the Recreation Binder.
Qualifications
- Bachelor’s degree preferred with two (2) year case management experience.
Or associate degree with four (4) year case management experience or two (2) years college with six (6) years case management and recreational experience.
*Prior experience preferred in case management, counseling with groups and/or individuals, crisis intervention and client advocacy. Knowledge of HIV-related intervention and substance issues necessary.

Pay: $37,400.00 - $37,500.00 per year
