About the Role:
As a MDS Case Management professional, you will be responsible for:
- Conducting comprehensive assessments of residents to identify their needs and develop individualized care plans
- Collaborating with healthcare professionals, including physicians, nurses, and therapists, to develop and implement care plans
- Coordinating with interdisciplinary teams to ensure that care plans are carried out effectively
- Documenting assessments, care plans, and progress in accordance with regulatory requirements
- Maintaining accurate and up-to-date records of resident care and progress
- MDS Case Management experience preferred
- Bachelor's degree in a related field (n/a)
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Familiarity with MDS guidelines and regulations
