What are the responsibilities and job description for the Practice Optimization Specialist position at RWJBarnabas Health?
As part of RWJBarnabas Health s Population Health team you will play an instrumental role in providing support to practices in achieving a model for Improving and managing Medical Group Performance in various Value Based Programs. A Practice Optimization Specialist s main objective is to support the Medical Group to efficiently deliver value to patients and physicians while operating as a high-performing academic medical group within the RWJBH One System-One Family Model. Practice Optimization teams will partner with the Primary Care Service Line to align transformation activities and allow for quick implementation and dissemination of information to practices.
PRIMARY RESPONSIBILITIES
- Interface with Practices and key members of the Population Health, Primary Care Service Line, and Physician Leadership Teams coordinating efforts to improve value-based performance within Medical Group practices.
- Collect and analyze data to provide practice facing teams with actionable targets
- Identify practice opportunities requiring focused effort and close gaps in care
- Review upcoming value base program opportunities and coordinate participation discussion with Service Line, complete contractual agreements.
- Conduct ongoing education on Epic workflows, Value Based Metrics, and other needs as they arise
- Host standing education platforms virtually or in person on a monthly basis
- Collaborate with the Service Line to develop and train detailed practice workflows to enhance value base performance
- Successfully work with practice manager to assist with change management efforts at the practice level around workflow redesign
- Serve as escalation point when workflow opportunities are identified in implementation practice workflow changes and addressing low performance
- Manages communication to the practices and practice staff around value-based activities
- Providing coding education and best practice around value-based programs and activities
EDUCATION
Associates or Bachelor s Degree in a healthcare related field preferred or equivalent experience working as a certified medical assistant for 3-5 years.
EXPERIENCE
Up to 75% travel within the state.
Experience with Epic required
Must possess, or within 30-days a valid New Jersey Driver s License or ID card and dependable transportation.
Onsite work with designated practices and limits remote work
Travel between practices required
Knowledge of population health is preferred
Previous work experience in physician practice is preferred
Medical Terminology required
Understanding HIPAA and ability to protect patient information
