  • Coordinating and overseeing Trauma Patient Navigator staff by assisting with hiring, delegating responsibilities, monitoring workflows, and supporting performance and professional development.
  • Monitoring, collecting, managing, and maintaining program data across multiple systems, including productivity logs, screening and intake documentation, scorecards, and other required reports.
  • Tracking program outcomes and presenting HVIP data during Social Impact, community partnership, and grant-related meetings.
  • Collaborating with hospital staff, community organizations, and interdisciplinary partners to promote coordinated care and improve access to services.
  • Providing direct victim advocacy and support services to Project HUDSON participants before transitioning clients to Trauma Patient Navigators for ongoing intensive case management.
  • Participating in required trainings, including hospital orientation, trauma-informed care education, and community outreach shadowing opportunities.
  • Completing timely documentation and maintaining accurate records in accordance with organizational policies, grant requirements, and program standards.
