RVNAhealth is seeking a dedicated Private Duty Registered Nurse (RN) to join our compassionate, high-performing team.
In this role, you'll provide direct patient care and case management for clients in their homes and in facility settings across Central and Southern Fairfield County, CT including Ridgefield, Wilton, Redding, Stamford, Norwalk and surrounding areas.
This is a Part-Time position with a schedule consisting of 10am-2pm, Monday-Friday, with a rotating on-call weekend.
Key Responsibilities
- Provide direct, hands-on nursing care to patients of all ages in the home and facility environment.
- Perform comprehensive assessments and identify patient and family needs.
- Develop, update, and maintain individualized nursing care plans in collaboration with physicians and the care team.
- Evaluate patient progress and promptly adjust care based on changes in condition or treatment orders.
- Counsel patients and families on health and wellness topics, including nutrition, safety, and prevention.
- Maintain accurate and timely documentation per agency and regulatory standards.
Qualifications
- Graduate of an accredited school of nursing.
- Current CT Registered Nurse (RN) license in good standing.
- Minimum 1 year of nursing experience (hospital or acute care preferred).
- Home Health or Private Duty experience strongly preferred.
- Valid driver's license, reliable transportation, and ability to travel throughout Fairfield County, CT.
- Strong communication, documentation, and critical thinking skills.
