What You'll Do
As a Home Health Physical Therapist, you will:
- Perform comprehensive physical therapy evaluations and collaborate with physicians, nurses, patients, and families to develop individualized care plans.
- Conduct OASIS assessments
- Deliver goal-oriented, evidence-based therapy in the home setting to help patients regain function and independence.
- Provide education and instruction to patients, families, and caregivers.
- Identify and refer for additional services or next level of care as needed.
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
- Connecticut PT license in good standing
- 2 years of clinical experience (home health experience preferred)
- Proficiency in OASIS documentation and EMR systems (tablet provided)
- Strong understanding of therapy best practices in home-based settings
- Excellent communication, time management, and problem-solving skills
- Valid driver's license and reliable vehicle for travel throughout Fairfield County
