Physical Therapist - Home Health (South)

We are hiring Physical Therapists for Full-time, Part-time or Per-Diem work schedules.

As a trusted community-based, nonprofit health agency, RVNAhealth provides high-quality care that supports patients where it matters most: at home. We are currently seeking a skilled and compassionate Physical Therapists to join our team and provide in-home therapy services across

Southern Fairfield County, including Norwalk, Stamford, Darien, Greenwich and surrounding towns.

What You'll Do

As a Home Health Physical Therapist, you will:

• Perform comprehensive physical therapy evaluations and collaborate with physicians, nurses, patients, and families to develop individualized care plans.
• Conduct OASIS assessments
• Deliver goal-oriented, evidence-based therapy in the home setting to help patients regain function and independence.
• Provide education and instruction to patients, families, and caregivers.
• Identify and refer for additional services or next level of care as needed.
  
  

What You Bring

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
• Connecticut PT license in good standing
• 2 years of clinical experience (home health experience preferred)
• Proficiency in OASIS documentation and EMR systems (tablet provided)
• Strong understanding of therapy best practices in home-based settings
• Excellent communication, time management, and problem-solving skills
• Valid driver's license and reliable vehicle for travel throughout Fairfield County
