RVNAhealth is seeking a compassionate and dependable Per Diem Licensed Practical Nurse (LPN) to provide high-quality, one-on-one patient care across 12-hour day and night shifts. Assignments may be short- or long-term and include hospice cases requiring around-the-clock medication administration and support with activities of daily living (ADLs).

Key Responsibilities
- Provide direct, hands-on nursing care to patients of all ages in a home setting
- Conduct comprehensive patient and family assessments to identify care needs
- Monitor patient condition and progress; promptly report and respond to changes
- Develop, update, and maintain individualized nursing care plans
- Administer medications and treatments in accordance with physician orders
- Educate patients and families on wellness topics including nutrition, safety, and overall health
- Utilize and maintain nursing equipment and supplies appropriately
- Communicate clinical findings to physicians, patients, and families as needed
- Identify patients at risk for neglect and initiate referrals to appropriate community resources
- Complete timely and accurate clinical documentation in compliance with agency and regulatory standards
- Provide coverage for evenings, weekends, and holidays as needed
Qualifications
- Graduate of an accredited school of nursing
- Active Licensed Practical Nurse (LPN) license in the State of Connecticut
- Minimum of 1 year of nursing experience (hospital experience preferred)
- Home health and/or hospice experience strongly preferred
- Strong clinical, communication, and documentation skills
- Ability to work independently in a home care environment
- Valid driver's license and ability to travel throughout the assigned territory.
Compensation:
The pay range for this position is $35.00 to $37.00 per hour and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
