Location: Lyndon, VT 05851
Contract Length: 13 Weeks
Shift: 12-Hour Nights (6:00 PM – 6:00 AM)
Guaranteed Hours: 48 Hours per Week
Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a 13-week contract assignment. The ideal candidate will provide high-quality nursing care to residents in a skilled nursing and rehabilitation setting while collaborating with an interdisciplinary healthcare team to ensure exceptional patient outcomes.
Responsibilities
- Provide direct nursing care to residents in accordance with facility policies and state regulations.
- Administer medications and treatments as prescribed.
- Monitor and document patient conditions, vital signs, and responses to treatment.
- Assist with resident assessments and care planning.
- Communicate effectively with physicians, nurses, residents, and family members.
- Respond promptly to changes in resident conditions and emergencies.
- Maintain accurate and timely clinical documentation.
- Support infection control and safety protocols.

- Facility Bed Capacity: 60 Beds
- Current Census: Approximately 57 Residents
- Three Resident Care Wings
Qualifications
- Current Licensed Practical Nurse (LPN) license in Vermont or compact state eligibility.
- Previous experience in long-term care, skilled nursing, rehabilitation, or geriatric nursing preferred.
- Current BLS/CPR certification.
- Strong clinical, communication, and documentation skills.
- Ability to work independently and as part of a multidisciplinary team.
