Community Care Network is looking for a Developmental Services Medical Care Coordinator to join our team in Rutland, Vermont.

About the Role:

The Developmental Services Medical Care Coordinator manages an assigned caseload of individuals with complex or ongoing medical needs and helps ensure continuity of care. This position works closely with individuals, guardians, Care Coordinators, Shared Living Providers, Direct Support Professionals, Developmental Services Nurse Consultants, medical providers, hospitals, pharmacies, and other members of the support team.

Key responsibilities include maintaining Emergency Fact Sheets (EFS), obtaining medical records, tracking appointments and medical recommendations, reviewing Medication Administration Records (MARs), and coordinating follow-up after significant medical events. This position works collaboratively with licensed nursing staff and does not replace clinical nursing assessment or judgment.

Principal Responsibilities:

  • Manage an assigned caseload of individuals with complex or ongoing medical support needs.
  • Coordinate and track primary care and specialty appointments, referrals, testing, and recommended medical follow-up.
  • Maintain accurate and current Emergency Fact Sheets (EFS), including updates following hospitalizations, medication changes, and other significant health changes.
  • Request, obtain, and organize medical records, appointment summaries, discharge paperwork, and provider recommendations.
  • Review Medication Administration Records (MARs) according to agency procedures; identify missing documentation or discrepancies and coordinate review with Developmental Services Nurse Consultants.
  • Assist with coordination following hospitalizations, emergency department visits, rehabilitation stays, and other significant medical events.
  • Communicate health-related information with individuals, guardians, Care Coordinators, Shared Living Providers, DSPs, Nurse Consultants, and medical providers as appropriate.
  • Identify barriers to accessing medical care and collaborate with the team to develop solutions.
  • Support providers and staff with health-related documentation and established medical follow-through expectations.
  • Collaborate with Nurse Consultants, Quality & Compliance, and DS Leadership to identify recurring documentation concerns, health trends, and training needs.
  • Complete all required documentation accurately and timely, including but not limited to: Individual Service Agreement (ISA) & attachments; home visits notes; contact notes; Critical Incident Reports (CIR); and Direct Service Organization (DSO) monthly summaries that include data from medication administration records and direct support data.
  • Maintain strong, collaborative relationships with providers to ensure individuals are safe and well-supported.
  • Facilitate clear and professional communication among all team members and partners.
  • Follow requirements related to confidentiality, HIPAA, mandated abuse and neglect reporting, Critical Incident Reporting (CIR).
  • Adhere to guidelines set forth by Department of Aging and Independent Living (DAIL) and Medicaid including but not limited to Health and Wellness Guidelines, Medicaid Manual, State System of Care, Local System of Care, Individual Support Agreement (ISA) and Behavior Support Guidelines (BSP).
  • Attend required meetings, supervision, and trainings and perform other duties as assigned.

Qualifications and Competencies:

  • 2 years of direct Medical experience preferred; Associate's Degree required; Bachelor's Degree in a related field preferred.
  • Eligible for Qualified Developmental Disabilities Professional (QDDP) status.
  • Previous experience in developmental disabilities preferred; experience with medical care coordination or complex medical needs is strongly preferred.
  • Strong organizational, documentation, communication, and follow-through skills.
  • Ability to manage multiple appointments, medical records requests, deadlines, and competing priorities.
  • Ability to recognize concerns requiring licensed nursing review and appropriately escalate those concerns.
  • Proficiency with computers, electronic medical records, and related technology.
  • Ability to maintain confidentiality and appropriately handle protected health information.
  • Flexibility, dependability, ability to work independently, and commitment to person-centered services.
  • Valid driver's license, reliable transportation, required automobile insurance, and successful completion of required background checks.
  • Must have a valid driver's license and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
    • Successful completion of a criminal history background check after hire.
