LPN Licensed Practical Nurse Per Diem
Job Summary:
We are seeking a compassionate and experienced LPN to join our team at Rutland Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider. As an LPN Per Diem, you will play a vital role in delivering exceptional patient care and supporting our mission to "Revolutionizing Care Delivery." As part of our per diem team, you will work on an as-needed basis, providing flexible and responsive care to our residents and patients.
Responsibilities:
Provide high-quality, patient-centered care to residents and patients, including administering medications, wound care, and other treatments as needed
Collaborate with interdisciplinary teams to develop and implement personalized care plans, ensuring coordinated and effective care delivery
Conduct routine vital sign checks, monitor patient conditions, and report concerns to RNs or other healthcare professionals
Participate in quality improvement initiatives and contribute to the development of best practices in patient care
Maintain accurate and up-to-date patient records, adhering to HIPAA guidelines and facility policies
Requirements:
Current LPN licensure in the state of Vermont
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
Ability to work flexible hours, including evenings, weekends, and holidays, on an as-needed basis
