Responsibilities
- Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or resident’s
as needed - Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure patient health and safety
- Support medication administration following HIPAA guidelines and care plans established by healthcare professionals
- Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness in kitchen areas
- Conduct patient observation to identify changes in condition or behavior that require attention or escalation
- Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met
- Document patient care activities accurately and report any concerns regarding resident well-being or safety to supervising staff
Qualifications
- Prior experience working with individuals with developmental disabilities, Alzheimer’s disease, dementia, autism spectrum disorder or in hospice care is highly desirable
- Knowledge of first aid, CPR certification, and basic nursing skills such as vital signs monitoring and medication administration
- Experience in group homes, assisted living facilities, nursing homes or home health environments preferred
- Ability to lift heavy objects safely and assist residents with mobility using appropriate equipment like Hoyer lifts or resident’s
- Strong communication skills with a compassionate approach to patient care and social work principles
- Familiarity with HIPAA regulations to ensure confidentiality of patient information
- Demonstrated ability to work independently as well as part of a team committed to delivering high-quality care
