Home Infusion Pharmacist - Seattle, WA

Two Clinical Infusion Pharmacist positions are available in a home infusion setting in Seattle, WA (Tukwila area), with a contract-to-hire opportunity starting ASAP. These roles require candidates with strong clinical backgrounds and home infusion experience to support a high-volume site serving approximately 1,300 patients, including 800 specialty patients. The site processes 350-400 prescriptions daily, with over 100 compounded medications.

Schedules are as follows:

• One opening: 9:00 AM to 5:30 PM, Monday through Friday, no weekends
• One opening: 10:00 AM to 6:30 PM, Monday through Friday, no weekends

New hires will undergo a standard two-week training period, followed by 3-4 months of clinical training to ensure full competency in the role. The pharmacy currently uses CPR and will transition to CareTend in September, with EPIC utilized for charting.

Key Responsibilities:

• Review and verify medication orders for accuracy, appropriateness, and compliance with regulatory standards
• Provide clinical oversight for infusion therapies, including specialty medications, biologics, antimicrobials, total parenteral nutrition (TPN), and compounded sterile preparations
• Assess patient-specific therapy plans and monitor outcomes to optimize treatment effectiveness
• Collaborate with physicians, nurses, and healthcare team members to guide medication management and therapy decisions
• Ensure adherence to state and federal pharmacy regulations, accreditation standards, and organizational policies

Qualifications & Desired Experience:

• Active WA Pharmacist license and local to Seattle, WA area
• Minimum 2 years of clinical experience in home infusion preferred; outpatient infusion considered
• Strong clinical background including inpatient, IV, TPN, vancomycin, biologics, and compounding
• Experience with specialty patient populations, including rare disease states and complex cases, is a plus
• Retail pharmacy background not suitable for this role
