Position Summary

The Referral Coordinator is responsible for activities and actions related to promoting the services and expertise of hospice to the medical community and to patients and family members.

Qualifications

  • Education in healthcare related field.
  • Certified, licensed, or registered as appropriate.
  • Two years of healthcare experience required. One year experience in hospice preferred.
  • Knowledge of hospice requirements including Medicare, Medicaid, and private insurance guidelines.
  • Good communications skills.
  • Ability to respond to the needs of physicians, discharge planners, patients and family members.
  • Ability to coordinate services with referral sources, hospice staff and patients/families.

Essential Functions And Responsibilities

  • Addresses inquiries and requests for hospice information.
  • Responds to referrals for evaluation and/or admission to hospice program.
  • Verifies and completes all admission and financial information and conducts chart audits.
  • Assures there is a valid physician order for assessment and admission.
  • Confers with clinical management regarding admission decisions.
  • Coordinates admission process between all team members.
  • Explain the hospice/palliative program to patients, family members, and other referral sources in an accurate, complete and thorough manner.
  • Provides statistical information to the Administrator as requested.
  • Assures efficient transition of patient unto hospice service, including but not limited coordination of equipment, supplies and transport.
  • Maintains continuing education through academic studies, seminars, and workshops.
  • Attends and participates in in-services, staff meetings and case conferences.
  • Completes all documentation in an accurate and legible manner.
  • Complies with hospice policies and procedures, laws and regulations and standards of practice.
  • Performs other tasks as may be assigned.
