We are seeking caring and dependable Certified Home Health Aides (CHHAs) to provide personal care and emotional support to hospice patients in their homes throughout the Greater Los Angeles area.
As part of our hospice care team, you will assist patients with daily living activities while helping maintain comfort, dignity, and quality of life.
Responsibilities
- Assist patients with personal care and hygiene
- Provide compassionate companionship and emotional support
- Support patient comfort and safety in the home setting
- Follow care plans directed by the RN Case Manager
- Document patient care accurately in EMR systems
- Communicate effectively with the hospice interdisciplinary team
- Current California CHHA Certification required
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to travel throughout assigned territories
- Hospice or home health experience preferred
- Bilingual English/Spanish is a plus, but not required
- Compassionate, dependable, and professional attitude
