Qualifications
- A Masters or Doctorate degree from a social work program accredited by the Council on Social Work Education.
- Will have a minimum of one (1) year experience in Social Work in a health care setting, preferably in the homecare setting.
- Currently licensed “in good standing” in VA.
- Must have a criminal background check.
- Must have current CPR certification.
Responsibilities
- Conduct thorough psychosocial assessments to evaluate patient needs, strengths, and barriers to care within the home environment
- Develop personalized care plans that incorporate community resources, mental health support, and end-of-life planning
- Provide individual, family, and group counseling sessions utilizing evidence-based practices such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy
- Coordinate with interdisciplinary teams including nurses, physicians, therapists, and community organizations to ensure holistic patient care
- Assist with discharge planning by identifying social determinants of health and facilitating access to services like hospice care, assisted living, or outpatient programs
- Maintain accurate medical documentation in compliance with HIPAA regulations and social work practice standards
- Support crisis intervention efforts and provide mental health counseling for patients experiencing acute emotional or behavioral health challenges
Skills
- Proven experience in medical social work within home health, hospice & palliative medicine, or acute care settings
- Strong knowledge of community resource coordination for diverse populations including geriatrics, pediatrics, and individuals with developmental disabilities or dementia
- Expertise in clinical counseling techniques such as psychotherapy, crisis management, and motivational interviewing
- Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management
- Ability to perform comprehensive patient assessments and diagnostic evaluations with sensitivity to cultural backgrounds
- Working knowledge of Medicare regulations and discharge planning procedures
- Excellent client communication skills paired with interprofessional collaboration abilities
- Certifications such as Basic Life Support (BLS), CPR, and training in hospice & palliative medicine are highly desirable
