Job Summary
We are seeking a compassionate and dedicated Caregiver / Provider to deliver exceptional support and assistance to individuals in various care settings. Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
- Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately
- Administer medications following care plans and HIPAA privacy regulations to ensure safety and compliance
- Prepare nutritious meals tailored to dietary needs and assist with feeding as required
- Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and sanitation of living spaces
- Support clients during outings or community activities to promote social engagement and independence
- Document care activities thoroughly in accordance with facility or agency protocols, including patient observations and progress notes
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group home environments
- Knowledge of dementia, Alzheimer’s, memory care, autism spectrum disorders, and working with individuals with developmental disabilities or disabilities in general
- Certifications such as First Aid, CPR, HIPAA compliance training preferred but not required; willingness to obtain necessary certifications is a plus
- Strong observational skills for patient monitoring and recognizing changes in condition or behavior
- Ability to perform heavy lifting safely when assisting with transfers or mobility needs
- Excellent communication skills for collaborating with healthcare teams, patients, families, and social workers
- Compassionate attitude combined with a proactive approach to delivering personalized patient care
