Job Summary:

The After Care Case Manager provides specialized aftercare case management services to youth and their families who have been discharged from an RMYA program. The aftercare case management is done by collaborating with the individual, their family, and their team. After Care Case Managers will develop and implement a plan that will help the youth and their families receive the resources needed to successfully obtain, transition, and remain in safe placement.

After Care Case Managers will be responsible for providing case support; intakes; discharges; and development, implementation, and review of therapeutic services for assigned residents in collaboration with the primary clinician. In addition, the After Care Case Manager will provide after-care support, follow-ups, and documentation after a youth has been discharged from RMYA. The position requires coordination with various government agencies, community groups, civic and non-profit organizations and RMYA resources to connect youth to needed services.

The Aftercare Case Manager will ensure that Aftercare services and activities are regularly reviewed and evaluated for effectiveness as part of the continuous quality improvement process. To include regularly receiving feedback from external stakeholders, youth and employees to adjust strategies as needed to improve outcomes.

Essential Duties/Responsibilities:

· Upon discharge (both successful and unsuccessful), the youth or young adult will exit

placement with a robust Aftercare Services plan (which may be incorporated as a part of the Service Plan) that at a minimum, includes the name and contact information for the STAR

Health Service Coordinator (if assigned) and the Youth Placing Agency’s Aftercare Services Case Manager, referrals for continued services, Education Portfolio, initial appointments set (if transition is needed), as well as a plan with times and dates set for twice a month follow-up calls and/or meetings for a period of 6 months. Additional in-person or virtual ad-hoc

meetings/staffing, as well as referrals for new or additional services, may be required during the 6-month after-care period.

· Collaboration with the youth’s core Service Planning team, which dependent on the case, should include: the youth, the youth’s parents, the youth’s CPS or SSCC caseworker, attorney ad-litem, guardian ad-litem and/or CASA volunteer, STAR Health Service Coordinator, relatives, subsequent Caregivers, and other stakeholders.

· Collection, documentation, and tracking of youth outcome data, related to the provision of Aftercare Services.

· Maintain written documentation of all contacts or attempted contacts, referrals, and other case management support provided during the Aftercare Service period in the General Residential Operation’s case record for the child.

· Prior to transition, administration, and completion of the CANS 3.0 Assessment. Review of assessment with Service Planning team members to identify strengths and needs to build on and address in subsequent placement.

· Assistance with identification, facilitation and support of affirming, normative, age-appropriate, positive-peer relationships, and activities within the youth’s community at the subsequent placement. Activities can include any number of things that are meaningful to the youth and contribute to positive well-being, which may include sports, fine arts,

volunteering, employment, extra-curricular, school activities, etc.

· Organization and facilitation of the transition to other medical and mental health providers, as needed. This includes collaboration to ensure that there is no lapse in therapy or

medication, if applicable.

· Assessment, assistance, and support of the needs of parents and/or subsequent Caregivers and family.

· Consistent and ongoing engagement with the youth and families to support transition and to maintain healthy connections.

· Assist in providing case management for youth who are currently in an RMYA program on assigned caseload.

· Serve as the point of contact for the youth with the treatment team about youth’s needs

and progress in care.

  • Collaborate with the Medical Consenter and document all services the youth is receiving through STAR Health, HHSC Behavioral Health, the juvenile justice and education systems, and any other county, community, or state agency.
  • Seek community resources to obtain any needed services that are not covered through STAR Health.
  • Conduct Quarterly Audits of Program Files and Clinical Files.

Intakes and Discharges

  • Coordinate pre-admission of youth, provide new youth orientation, ensure intake documentation meets compliance requirements, completes admission assessments, and preliminary service plans in client management system.
  • Complete admission paperwork for youth being placed with RMYA.
  • Coordinate and oversee discharges for youth on caseload.
  • Must be able to maintain a flexible schedule for intakes and discharges 24/7.

Relationships and Collaboration

  • Work in collaboration with program and all other agency staff to facilitate a team environment; participates in team meetings and commits to group decisions; role models effective team behaviors; demonstrates effective communication skills in building relationships with all employees and clients.
  • Foster teamwork by working collaboratively and effectively with others to set goals, resolve problems, and ensure decisions are made in the best interest of the youth, families and the agency.
  • Contribute ideas and solutions to the Compliance and Quality Improvement Plan.
  • Protect the confidentiality of clients and RMYAs proprietary information.
  • Adhere to RMYAs Code of Ethics.
  • Maintain Compliance with all training requirements.
  • Maintain compliance with all RMYA, DFPS, T3C, SSCC and COA policies and procedures and with all federal, state and local laws.
  • Complete documentation, reports and responsibilities within assigned deadlines.
  • Perform other duties as assigned.

Required Qualifications:

  • Bachelor’s degree in social science or related field required; master’s degree in social

science or related field preferred.

  • 2 years case management experience required. Previous experience working with clients in a residential, shelter, community based, or hospital setting preferred.
  • Deep Understanding of Department of Family Services (DFPS) licensing and operating requirements preferred.
  • Certification/Licenses: Valid Texas driver’s license, Military ID, or ability to obtain within 90 days of hire. Must have an acceptable driving record.
  • Successful clearance of Department of Family and Protective Services licensing background checks, (to include fingerprinting and drug testing) and maintenance of cleared status.
  • Ability to maintain compliance with applicable Texas rules, regulations, and laws that govern behavioral and mental health services for youth and their families ensuring that the highest standards of generally accepted clinical, ethical, and professional standards are followed.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.

  • Ability to bend, rotate arm and wrist, climb stairs, push, pull, reach, write, and type.
  • Ability to lift and carry at least 10 pounds.
  • Ability to drive.
  • In person attendance with face shield/mask required.
