JOB SUMMARY:
The Family Practice NP/PA provides comprehensive primary care services to patients across the lifespan, including preventive care, acute illness management, and chronic disease management. The provider works collaboratively with physicians and healthcare staff to deliver high-quality, patient-centered care in an outpatient family practice setting.
ESSENTIAL FUNCTIONS:
Clinical Duties
- Perform comprehensive history and physical examinations for patients of all ages
- Diagnose and treat acute and chronic medical conditions
- Develop and implement individualized treatment plans
- Order, perform, and interpret diagnostic tests (labs, imaging, EKGs, etc.)
- Prescribe medications and therapies in accordance with state regulations
- Provide preventive care services including annual physicals, immunizations, and screenings
- Manage chronic conditions such as hypertension, diabetes, asthma, and hyperlipidemia
- Provide women’s health services (e.g., Pap smears, contraceptive counseling) as appropriate
- Perform minor office procedures (suturing, incision & drainage, joint injections, etc.)
- Provide patient education regarding diagnoses, treatment plans, lifestyle modifications, and disease prevention
Care Coordination
- Collaborate with supervising physician and interdisciplinary team
- Refer patients to specialists and community resources when appropriate
- Follow up on diagnostic results and ensure continuity of care
- Participate in care management programs and quality improvement initiatives
Documentation & Compliance
- Accurate document of patient encounters in the Electronic Health Record (EHR)
- Maintain compliance with HIPAA and regulatory requirements
- Adhere to clinical guidelines and evidence-based practice standards
- Participate in peer review and performance improvement activities

Qualifications:
Education / License / Certifications:
- Current state licensure as NP or PA
- National board certification (e.g., AANP, ANCC for NPs; NCCPA for PAs)
- Current DEA registration
- BLS (ACLS preferred) certification
- Previous experience in family practice, primary care, or internal medicine preferred
- New graduates may be considered depending on training and supervision structure
Language Skills: Ability to understand written sentences and paragraphs in work related documents.
