What are the responsibilities and job description for the Founding Engineer (AI / Full Stack) position at Rovi Health (YC F25)?
You’ll be one of the first engineers at Rovi and own large parts of the product end-to-end. You’ll talk to customers, decide what to build, and ship it. You’ll work directly with the founders, set the engineering bar, and make product decisions that shape the trajectory of the company.

What You'll Build
AI agents that make care effortless\ Someone texts us with a symptom. The agents you build run clinical interviews, figure out what care they need, find the best provider based on price/quality/convenience, book the appointment, and follow up. These agents scale across thousands of people and dozens of conditions.
Predictive systems that catch problems early\ We have access to claims data, clinical records, and real-time patient interactions. You’ll build pipelines that spot patterns like missed follow-ups or avoidable ER visits and trigger interventions before they turn into $50K hospital stays.
Systems that drive efficiency and engagement\ Build the patient chat that gets people to use Rovi instead of calling scheduling lines, and the clinician workflows that let one doctor handle 10x more patients asynchronously. You’ll talk to users, figure out what actually changes behavior, and ship improvements quickly.
Infrastructure that scales\ We’re serving thousands of users today and heading toward hundreds of thousands. You’ll build the systems, data models, and integrations that can support that scale while still letting us move fast now.

The root problem is that healthcare isn’t a real market. Prices and quality are opaque. Patients don’t shop; they follow default referrals. Hospitals charge whatever they can negotiate. Employers outsource cost control to insurers who profit when premiums rise.
Rovi fixes this by creating a demand curve.
We make it radically easier and cheaper to get high-quality care. We find the best in-network doctors based on price, quality, and convenience, and reimburse copays when people follow our recommendations. As more people use Rovi, we steer demand at scale.
The end state is an efficient healthcare market with Rovi as the rails: routing demand to the right providers, reshaping supply around value, and cutting costs by 30% while improving outcomes.

What We're Looking For
- 3-7 years of engineering experience, strong in Python, React and TypeScript
- Full-stack experience across frontend, backend, data infrastructure, and ideally some AI workflows
- Experience building automation or agentic systems in production
- Comfortable with cloud environments (AWS, GCP, or Azure)
- Strong PostgreSQL / SQL skills and solid database design
- A track record of shipping user-facing products end-to-end, not just pieces of a system
- You’re motivated by making healthcare more affordable and accessible for millions of people
