Responsibilities
- Participates in the development of individual service plans for new residents and ongoing review/updating of individual service plans in response to changes in resident needs.
- Participates in the completion of resident assessments.
- Ensures resident charts are current and organized and required documentation is completed.
- Assists with admissions and discharges to ensure coordination of services.
- Performs routine reviews of physician orders for accuracy.
- Functions as liaison with families, physicians, and related healthcare agencies regarding care/ health and wellness issues for residents.
- Models teamwork and demonstrates the ability to interact positively with a caring, nurturing, and professional attitude.
- Other duties as assigned.
Qualifications
- Current RMA or RN/LPN license
- Current adult CPR certification
- Excellent customer service and interpersonal skills
- Knowledge of or willingness and ability to learn computer programs.
- UAI/ISP certification or willingness to obtain certification within 60 days of hire.
