Overview:
We are seeking a dedicated MDS Coordinator to join our team. The ideal candidate will have experience in nursing homes and possess strong skills in medical records management, patient assessment, and urgent care. Potential hybrid position for seasoned candidates.
Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments
- Ensure accurate and timely documentation of patient medical records
- Conduct patient assessments including vital signs monitoring and behavioral health evaluations
- Collaborate with healthcare providers to develop individualized care plans
- Assist in catheterization, tube feeding, and patient lifting as needed
- Provide urgent care support when required
Experience:
- 3 years of reimbursement or similar experience in the LTC industry
- Proficiency in medical records management and documentation
- Ability to conduct patient assessments and monitor vital signs
- Computer literacy (Word, Excel, PowerPoint, Outlook)
- Working knowledge of state and federal regulations governing the MDS and billing processes within a nursing facility
- Ability to apply knowledge in state specific Medicaid and Medicare RUG/PDPM methodologies
