Position Purpose: The Transition Planner develops a continuing aftercare plan, based on the client’s needs, to promote long term health and relapse prevention. Responsible for setting discharge appointments for patients, including group or individual therapy, psychologist or psychiatrist. Makes arrangements for community resources to facilitate discharge. Provide all needed documentation to other providers to facilitate the continuum of care. Act as a liaison between referral sources, patients and counselors. Ensure all patient discharge planning documentation is clear, concise and timely.

Qualifications/Basic Job Requirements
- Bachelor’s Degree and one years’ experience in Behavioral Health Care
- Experience in the research and development of successful Behavioral Health discharge plans a plus
- Previous experience with area community programs and resources a plus
- Excellent customer service and conflict resolution skills
- Exceptional verbal and written communication skills. Must be able to communicate clearly, thoroughly, and professionally even when under stress and always maintain composure with clients, co-workers and all levels of Rosecrance management
- Proficient skills with EHR / EMR software, Microsoft Office (e.g., Word, Excel, Access, PowerPoint) are required, CRM applications a plus
- Proven excellent organizational and time management skills with the ability to meet multiple deadlines
- Work scheduled hours and any additional hours deemed necessary by leadership. Some variation in scheduled hours may be required to meet company needs.
- Ensure all appropriate discharge appointments for clients, including group or individual therapy, psychologist, or psychiatrist are set.
- Ensure all needed documentation to other providers to facilitate the continuum of care and all client discharge planning documentation is clear, concise, and timely.
- Serve as a member of the daily inter-disciplinary treatment team. Coordinate development and documentation of the Treatment Plan based upon the needs of the clients. Coordinate discharge with the treatment team and ensure that appropriate clients receive follow up within 7 days of discharge and documents according to Rosecrance standards.
- Meet with families as needed during the treatment process to explain continuing care, procedures, and process.
- Assist clients and families with the identification of a support network, instructions for future care, follow-up and concerns, and requests regarding care upon discharge.
- Assist client and families with completion of all clinical documentation needed for transition to their next programs or social service organizations.
- Maintain and protect client confidentiality in accordance with all relevant regulations; maintain confidentiality related to information being processed, stored, or accessed.
- Effectively manage assigned case load in a fast-paced environment while juggling multiple priorities.
- Exhibit action and make decisions that align with the Rosecrance Code of Conduct and within the framework of the law.
- Demonstrate strong customer service skills that incorporate the 5 tenets of the Rosecrance Way into daily practices and interactions with all stakeholders.
- Serve as a role model to others and demonstrate excellent company stewardship.

- Starting Pay: $24.44hr ($50,835.20 annually).
