Position Purpose: Provide continuity of program services for children and adolescents in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the delivery of addictions and mental health treatment services to patients and their families and facilitate the issues of recovery into their daily living situations.
Qualifications/Basic Job Requirements:
LCPC, LCSW, or LMFT preferred,
Licensed in Illinois as an LSW, LPC, or AMFT required.
Computer proficiency required.
Ability to navigate and document in electronic healthcare records required.
Ability to accurately complete required documentation within the time frames
Skilled in conducting individual, family and group counseling
Ability to work within a team

Essential Responsibilities:
Conduct individual, group, and family counseling and educational sessions for clients and/or family members.
Complete client psychosocial and individualized treatment plan within required time frame.
Identify client clinical and case management needs and work to address those needs.
Educate client and client support members regarding all aspects of treatment, corresponding health issues and steps to recovery.
Undertake primary case management responsibilities for designated clients.
Ensure that documentation in client service record is maintained consistent with expectations defined by licensing, accreditation, and funding requirements.
Maintain communications with all staff (e.g. admissions, case management and discharge plans).
Present workshops, lectures and training to clients, families, and staff and community groups.
Serve as a member of the team and participate in all team meetings and activities.
Exercise confidentiality in keeping with the Code of Conduct and with the framework of the law.
Deliver exceptional customer service consistently to every customer.
Serve as a member of the Clinical Team and participate in all team meetings and activities.
Serve as a role model and demonstrate positive guest relations in representing the agency.
Schedule: 8-hour shifts or part time hours
Shift:
Monday - Friday Flexible. Required at least 3 afternoon-evening shifts per week. 4 Ten hour shifts available as well. Part time hours available as well.
