Position: Pediatric Home Health RN/LVN Field Nurse

Schedule: Days/Nights/Weekends – Flexible
Compensation: $24 to $40 per hour

Position Summary

We are seeking compassionate, dependable, and clinically skilled Registered Nurses (RN) and Licensed Vocational Nurses (LVN) to provide skilled nursing care to pediatric patients in their homes.

The Pediatric Home Health Field Nurse provides individualized, family-centered care to children with complex medical needs, chronic conditions, developmental disabilities, and/or technology-dependent needs. The nurse works closely with the child, parents/caregivers, physicians, therapists, and the home health clinical team to promote safety, comfort, health, and quality of life.

The ideal candidate is patient, caring, dependable, and comfortable providing one-on-one nursing care in a home environment.

Essential Responsibilities

• Provide skilled nursing care to pediatric patients in their homes according to physician orders and the individualized plan of care.
• Perform nursing assessments appropriate to the patient's age, diagnosis, developmental level, and medical needs.
• Monitor vital signs, symptoms, clinical status, and response to treatment.
• Recognize and promptly report changes in the child's condition to the appropriate supervising nurse and physician/provider.
• Administer medications and treatments as ordered and within the nurse's scope of practice.
• Provide appropriate pediatric nursing interventions based on the child's diagnosis and plan of care.
• Provide care and monitoring for children with complex medical needs as applicable, including feeding tubes, tracheostomies, respiratory support, oxygen, suctioning, and other skilled nursing needs.
• Assist with enteral feeding and medication administration through feeding tubes when ordered and within the nurse's scope of practice.
• Perform wound care, catheter care, medication management, and other ordered treatments as applicable.
• Maintain a safe environment for the pediatric patient and identify potential safety risks within the home.
• Provide age-appropriate care that promotes the child's comfort, dignity, development, and independence.
• Educate and support parents and caregivers regarding the child's care, medications, treatments, safety, and disease management.
• Follow the physician's orders and established plan of care.
• Communicate effectively with the supervising RN, physicians, therapists, caregivers, and other members of the healthcare team.
• Document all nursing services, assessments, interventions, and patient responses accurately and timely in the agency's EMR.
• Follow infection prevention and control procedures, emergency procedures, HIPAA requirements, and agency policies.
• Maintain professional boundaries and respect the family's home, privacy, culture, and preferences.
• Participate in care coordination and case conferences as requested.
• Perform other nursing duties within the individual's scope of practice as assigned.

RN Responsibilities

The RN may be responsible for:

• Performing comprehensive nursing assessments and evaluating the pediatric patient's ongoing needs.
• Developing, coordinating, and evaluating the nursing plan of care as appropriate.
• Performing required comprehensive assessments and OASIS documentation when applicable to the patient's episode of care and the nurse's role.
• Providing clinical oversight and coordination of patient care.
• Communicating with physicians and other healthcare providers regarding the patient's condition and plan of care.
• Supervising LVNs and other clinical personnel as required by applicable regulations and agency policy.
• Providing clinical guidance to caregivers and members of the care team.
• Participating in quality assurance and interdisciplinary care coordination.

LVN Responsibilities

The LVN will:

• Provide nursing care under the direction and supervision required by applicable Texas law, physician orders, and agency policy.
• Follow and implement the established plan of care within the LVN's scope of practice.
• Monitor the pediatric patient's condition and promptly report changes to the supervising RN, physician, or other appropriate provider.
• Administer medications and perform treatments within the LVN's scope of practice and according to physician orders.
• Provide patient and caregiver education appropriate to the LVN's role.
• Accurately document all care provided and communicate patient progress and concerns to the clinical team.

Pediatric Qualifications – RN

• Current, unrestricted Texas RN license.
• Current CPR/BLS certification.
• Pediatric nursing experience required.
• Pediatric home health experience strongly preferred.
• Experience with medically complex children is a plus.
• Experience with G-tubes, tracheostomies, suctioning, oxygen, ventilators, or other pediatric medical equipment is a plus.
• Strong assessment, clinical judgment, communication, and documentation skills.
• Ability to work independently in a patient's home.
• Reliable transportation.
• Valid driver's license and appropriate automobile insurance.

Pediatric Qualifications – LVN

• Current, unrestricted Texas LVN license.
• Current CPR/BLS certification.
• Pediatric nursing experience preferred.
• Pediatric home health experience preferred.
• Experience caring for medically fragile or medically complex children is a plus.
• Strong clinical, communication, and organizational skills.
• Ability to follow an individualized plan of care and work collaboratively with the supervising RN and healthcare team.
• Reliable transportation.
• Valid driver's license and appropriate automobile insurance.

Preferred Pediatric Experience
