The Advanced Practice Provider (APP) provides comprehensive outpatient medical care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation.
They coordinate services within a Medical Home model, developing treatment plans that incorporate behavioral health, pharmacy, nursing, therapy, nutrition, or surgery, and monitor patient progress with referrals as needed.
Work is based at the Prineville Clinic, with a minimum of 40 hours/week, including scheduled call rotations covering evenings, weekends, and holidays, supported by backup physicians.
Key duties include delivering evidence-based family medicine, prescribing medications, interpreting diagnostics, educating patients, and ensuring documentation accuracy.
Qualifications include a Master’s in Physician Assistant Studies or Nursing, licensure, certifications, outpatient experience, and teamwork skills. Preferred candidates have 2 years’ experience, familiarity with Medical Home models, and Epic EHR proficiency.
