Overview
We are seeking a highly skilled and compassionate Functional Medicine Nurse Practitioner to join our healthcare team. This role offers an opportunity to provide personalized, holistic patient care within a dynamic clinical environment. The ideal candidate will possess extensive experience in patient care, medical documentation, and advanced clinical procedures, with a strong focus on integrative medicine approaches. Familiarity with EMR/EHR systems such as Epic, Cerner, and eClinicalWorks is essential. The Nurse Practitioner will collaborate with physicians and other healthcare professionals to deliver comprehensive assessments, develop treatment plans, and support patient education across diverse populations, including geriatrics, pediatrics, and patients with complex chronic conditions.
Responsibilities
- Conduct thorough patient assessments including vital signs, physical examinations, and detailed medical histories
- Develop and implement individualized treatment plans incorporating functional medicine principles
- Perform clinical procedures such as IV infusion, BHRT and injections
- Manage acute and chronic pain conditions.
- Utilize EMR/EHR systems for accurate documentation of patient encounters and medical records
- Coordinate diagnostic evaluations including laboratory tests, medical imaging, sonography, and specimen collection/processing
- Provide health coaching focused on nutrition, lifestyle modifications, and disease prevention
- Engage in telehealth consultations to expand access to care while maintaining HIPAA compliance
- Ensure infection control practices are followed rigorously using aseptic techniques and sterile processing protocols
Requirements
- Valid Nurse Practitioner license with certification in family medicine; additional certifications in urgent care or emergency medicine preferred
- Proven experience working in diverse clinical settings including hospital medicine, ICU (Level I & II trauma centers), emergency rooms, outpatient clinics, or post-acute care environments
- Extensive knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and medical-surgical procedures
- Proficiency with EMR/EHR systems.
- Ability to perform advanced clinical procedures.
- Experience working with hormone replacement therapy and functional medicine.
- Excellent patient assessment skills combined with compassionate communication abilities to educate patients effectively on health management strategies
