What are the responsibilities and job description for the Foster Care Case Manager position at Rooted in Community?
Overview
At Rooted in Community, Foster Care Case Managers play a key role in supporting youth and families involved in the foster care system. Foster Care Case Managers work collaboratively with families and service providers to ensure the safety, well-being, and permanency of children in care. This position requires initiative, advocacy, strong case management skills, the ability to create and monitor treatment goals, strong writing skills, and an in-depth understanding of the foster care system.
Responsibilities
- Develop individualized case plans for children in foster care, aligning with their unique needs and circumstances.
- Conduct assessments to review needs and create measurable goals which are guided by youth and families.
- Collaborate closely with community agencies to coordinate services and support networks. Strong advocacy needed to ensure continuity of services are addressing the barriers to the families needs.
- Coordinate family time and sibling visitation in a setting that represents the most family-like setting, may be required to supervise visits.
- Provide ongoing support and guidance to foster families, addressing challenges proactively and fostering positive relationships.
- Monitor children's progress through regular visits, documentation, and updates to ensure their developmental and emotional needs are met.
- Maintain detailed case records, compliance documentation, and reports in accordance with agency policies and legal requirements.
- Attend court hearings and provide professional testimony, presenting case information in a clear, objective, and respectful manner.
Requirements
- Minimum of a bachelor's degree from an accredited college or university with a major in a human behavioral science (Social Work, Criminal Justice, Psychology or similar), or in another major where 25% of the course credits earned toward the degree are in human behavioral sciences.
- Meet State of Michigan Good Moral Character Guidelines.
- Excellent communication skills to effectively collaborate with families, agencies, and community partners
- Ability to handle sensitive situations with empathy, professionalism, and transparency.
- A valid driver’s license and willingness to travel for home visits and meetings.
- Preferred Experience: past work in child welfare, childcare, or social work settings.
- Preferred Experience: Knowledge of child protective services protocols and best practices in foster care management.
