Job Summary
Provide professional social work services to patients and families in an ambulatory clinic setting. Assess psychosocial needs, develop care plans, provide counseling, crisis intervention, and coordinate resources to support optimal health outcomes. Work as a key member of the interdisciplinary care team to ensure patients receive comprehensive and compassionate care.
Essential Job Responsibilities
- Promote the mission, vision, and values of the organization
- Conduct psychosocial assessments and develop individualized care plans
- Provide therapeutic counseling to patients and families to address behavioral health, adjustment to illness, grief, and other psychosocial concerns
- Assist patients and families with resource identification, referrals, and coordination of community services
- Collaborate with physicians, nurses, and other healthcare professionals to support integrated care
- Provide crisis intervention and support in emergency situations
- Maintain accurate and timely documentation in the medical record
- Participate in discharge planning and ensure continuity of care
- Educate patients and families on coping strategies, behavioral health topics, and available resources
- Uphold ethical and professional standards in patient care and documentation
- Adhere to state and federal regulations, as well as hospital and clinic policies
- Participate in quality improvement initiatives and staff education as requested
- Seek supervision and guidance as needed, while also serving as a resource to other staff
- Demonstrate strong communication skills, cultural sensitivity, and professional behavior in all interactions

Qualifications
- Master’s degree in Social Work (MSW) from an accredited program
- Current license in the state of New Mexico as a Licensed Clinical Social Worker (LCSW)
- Minimum of 1–2 years of post-licensure clinical experience in behavioral health, hospital, or ambulatory care setting preferred
- Current Basic Life Support (BLS) certification required or must be obtained within thirty (30) days of hire
- Advanced Cardiac Life Support (ACLS) certification preferred
- Pediatric Advanced Life Support (PALS) certification preferred
