Care Coordinator

JOB SUMMARY

The Care Coordinator provides essential recovery-oriented support services to individuals within the Detention Center environment who are navigating mental health challenges, substance use issues, or co-occurring disorders. Additionally, the Care Coordinator will assist individuals with re-entry supports, including, but not limited to: transitional housing, employment, transportation, and continuum of care.

ESSENTIAL FUNCTIONS

The Care Coordinator plays a vital role for individuals in recovery, and will engage and interact with an unbiased, culturally sensitive approach within the confines of the facility, as well as within the community. Emphasis will be placed on successful community reintegration of individuals, upon release, reducing the instance of recidivism.

· Team Collaboration and Documentation: Working cooperatively as part of a multidisciplinary team, communicating effectively with clinical staff and other professionals, and accurately documenting services provided while maintaining confidentiality and adhering to ethical guidelines.

· Support Services: Provision of individual-specific discharge planning and re-entry supports to include facilitating phone calls, circulating emails, scheduling assessment and appointments, and ensuring warm hand-offs.

· Program Facilitation: Educate and prepare program participants for community reintegration, utilizing the ‘Living Skills’ Modules, SUCCEED, job finding, and any other identified tools, resources, or courses, which will train, educate, and prepare individuals with developed coping skills, identification of triggers, and positive outcomes, upon community reintroduction.

· Program Development: Observe, document, and report individual and group outcomes, and identify challenges and obstacles which may be barriers to success.

Non-Essential Functions

· Perform additional duties as assigned.

· Attend other meetings and/or trainings as assigned.

Professional Requirements

· Following a conditional offer of employment, successfully complete background investigation which may include, but is not limited to: fingerprinting, criminal record search, driving record screening, reference check, employment verification, and credit check.

· Meet physical fitness screening standards as required by Roosevelt County;

· Successful completion of required training, including but not limited to Cardiopulmonary Resuscitation (CPR), within one year from date of hire and Defensive Driving within six (6) months of date of hire.

· Complete annual education requirements as assigned or designated by certification or accreditation requirements.

· Adhere to dress code.

Report to work on time and as scheduled.

· Represent the County in a positive and professional manner at all times, including use of tact when working with a wide range of individuals, including the public, often under tense circumstances.

· Attend regular staff meetings and in-services.

· Be subject to random drug/alcohol tests.

· Participate in performance improvement and continuous quality improvement activities.

Qualifications

· Lived experience with mental health and/or substance use challenges, or equivalent education.

· High school diploma or equivalent (while some organizations may prefer additional education, this is the minimum educational requirement).

· Strong communication and interpersonal skills to effectively engage with clients and team members.

· Ability to work both independently and collaboratively as part of a multidisciplinary team.

· Basic computer and documentation skills for research, report writing, and accurate record-keeping.

· Understanding of and strict adherence to Roosevelt County Detention Center Policy & Procedure.

· Previous experience working in healthcare management working with the substance use disorder, mental health disorders, and/or co-occurring disorders population(s).

• Previous Correction/detention facility experience preferred. Related work experience desirable.
• Has not been released or discharged under dishonorable conditions from any of the armed forces of the United States.

· Is not currently on probation.

· Knowledge of investigative techniques and practices.

· Must be a U.S. citizen and a minimum of 21 years of age.

· Must possess a valid driver’s license.

· Bi-lingual preferred.

· Veterans preferred.

Knowledge, Skills, and Abilities

· Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.

· Must be alert at all times; pay close attention to details. Ability to handle stress and respond appropriately in stressful situations.

· Must be able to work under stress on a regular or continuous basis.

· Is knowledgeable about care and treatment needs of patients of different ages. Provides individualized care and treatment that is consistent with/sensitive to the age and life span development needs of each patient.

· Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.

· Must comply with safety guidelines and policies of the County.

· Understand and use Project Management.

Physical Requirements and Environmental Conditions

• Work irregular hours including after hours, with a minimum of thirty hours per week.
• Work under stressful conditions.

· Work in varying degrees of temperature (heated or air conditioned).

· Position requires light to moderate work with 50 pounds maximum weight to lift and carry.

· Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing.

· Manual and finger dexterity.

· Hand and eye coordination.

• Corrected vision and hearing to within normal range.
• Ability to sit for up to four (4) hours at one time, and up to eight (8) hours total per day, with opportunities to change positions.
• Worker may perform duties in an area of poor ventilation. Worker may be exposed to odors due to poor hygiene of probationers.
• Hazards or potential hazards of the job included high stress levels, dealing with a variety of probationers, being exposed to mental or physical health problems and the potential of retaliation from present or former probationers.

Pay: $20.00 - $23.00 per hour
