POSITION DESCRIPTION

Medical Transporter/Van Driver


BASIC FUNCTION


Assist the Nursing Department by scheduling outside appointments for clients to health care providers as directed. Transport and escort client’s to the appointments safely and timely. Ensure the Nursing Department is informed of recommendations of health providers. Acts as a liaison between the facility and outside health provider’s. Provide supervision and training to the client’s including training in health programs. Assist the Nursing Department to ensure smooth operation of department and facility. Follow all established Federal, State and facility policies/rules/regulations.


CHARACTERISTIC DUTIES AND RESPONSIBILITIES

ESSENTIAL FUNCTIONS

1. Schedule appropriate appointments in a timely fashion to ensure health care needs are met. Be careful not to over schedule so all clients can be served in a timely manner, without causing overtime for personnel.

2. Provide safe and timely transportation and escort to clients going on appointments outside the facility.

3. Clean inside of transportation van after each use to ensure trash and garbage are removed.

4. Conduct, document and assist with programs and activities for each client as specified by Individualized Program Plans, as directed by the Director of Nursing and/or Assistant Director of Nursing.

5. Supervise, observe, account for, and interact appropriately with clients in your care.

6. Represent the facility in a pleasant and professional manner with outside vendors, physicians, hospital staff, and other health care providers.

7. Provide physical care assistance to clients and document all required information pertaining to client’s physical care. Ensure nursing staff is informed of pertinent information.

8. Must keep a current appointment book with all documented appointments scheduled for clients.

9. All canceled and/or postponed appointments must be pre-approved by the Director of Nursing and/or Assistant Director of Nursing. These cancellations and/or postponements must also be done in a timely manner that is in compliance with regulations and facility policies.

10. Assist/ensure client’s appearance and personal hygiene is appropriate and acceptable before leaving facility for appointments.

11. Post and/or inform facility staff of client’s appointments in advance. Check with appropriate personnel regarding client’s schedule for Staffings, family visits, home leaves, etc.

12. Perform required housekeeping duties, related tasks and run errands in an acceptable manner.

13. Perform and/or assist other staff in an effective, professional, cooperative manner when working in conjunction with supervisor, peers, and professional or supportive staff in all duties required to provide quality care, programs, and services in a pleasant environment for clients.

14. Filing and record keeping will be done in a timely and accurate fashion as directed by the Director of Nursing and/or Assistant Director of Nursing.

15. Follow daily work schedule or supervisor’s instructions.

16. Performs other duties as deemed necessary and appropriate, or as may be directed by the Director of Nursing, Assistant Director of Nursing and/or Administrator.

MARGINAL FUNCTIONS: Attends and participates in inservices, workshops, etc.


EXPOSURE RISK: The Medical Transporter is at minimum risk for exposure to blood and body fluids.


SUPERVISION RECEIVED: Receives supervision from the Director of Nursing and/or Assistant to DON.


QUALIFICATIONS

  • EDUCATION: Must possess a high school diploma or equivalent,
  • EXPERIENCE: None required. On-the-Job training provided.

WORKING CONDITIONS

1. Works in well-lighted/ventilated areas.

2. Sitting, standing, bending, lifting (no more than 25% of your own body weight) and moving intermittently during working hours.

3. Subject to frequent interruptions.

4. Involved with patients/residents, family members, personnel, visitors, etc.

5. Subject to hostile and emotionally upset patients/residents, family members, etc.

6. Communicates with other departments and personnel if necessary.

7. Willingness to work beyond normal working hours, and in other positions temporarily, when deemed necessary.

8. Is subject to falls, burns, infectious diseases, substances, odors, etc., throughout the workday.


SPECIFIC REQUIREMENTS

1. Must be able to read, write, and speak the English language in an understandable manner.

2. Must possess good public relations skills.

3. Must possess the ability to deal tactfully with personnel, outside healthcare providers, clients, visitors, and family members.

4. Must be of good moral character.

5. Must be able to follow written and oral instructions and have a memory for details.

6. Ability to work harmoniously with other personnel, as well as, be willing to handle clients based on whatever maturity level they are currently functioning.

7. Must have personal integrity, have flexibility, and the ability to work effectively with other personnel.

8. Must maintain a clean, neat, modest, and professional appearance at all times.

9. Must be able to be insured through the facility’s vehicular insurance and possess a state Class “D” driver’s license.

10. Must possess the ability to make independent decisions when a circumstance warrants such actions.

11. Must be willing and able to go on out-of-town and overnight appointments independently.


PHYSICAL REQUIREMENTS: (With or without the aid of mechanical devices)

1. Must be able to move intermittently throughout the workday.

2. Must be able to speak English and communicate effectively with others.

3. Must be able to cope with the mental and emotional stress of the position.

4. Must be able to see and hear, or use prosthetic devices to enable these senses to function adequately to ensure that the requirements of this position can be fully met.

5. Must function independently, have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and healthcare providers.

6. Must be in good general health and demonstrate emotional stability.

7. Must be able to relate to and work with ill, disabled, intellectual disabilities, emotionally upset, and at times hostile people within the facility.

8. May be required to lift patients/residents, medical equipment, supplies, no more than 25 lbs.
