CAREGIVER PROGRAM CONSULTANT
ROCKLAND COUNTY OFFICE FOR THE AGING
The Rockland County Office for the Aging offers vital programs and services to support older adults and their caregivers throughout Rockland County.
The Caregiver Program Consultant is a contracted role within the Office, tasked with managing and executing the caregiver support program under the direction of the agency. This position focuses on coordinating services for eligible caregivers in Rockland County. Responsibilities include assessing caregiver needs, developing personalized care plans, connecting individuals with appropriate resources, and authorizing services. The consultant plays a key role in strengthening and delivering high-quality support to caregivers and their loved ones in the community.
Responsibilities Include:
· Contact individuals referred to the Caregiver Program.
· Determine an individual’s eligibility for caregiver services as per program regulations.
· Organize caseload and set priorities appropriately.
· Conduct caregiver assessment and reassessment using a prescribed assessment tool.
· Determine services available to best meet identified needs.
· Work with caregivers to develop an individualized caregiver support plan.
· Complete authorization process for caregiver program services.
· Provide periodic follow-up to ensure satisfaction with services.
· Enter client information, notes and service units into a database.
· Maintain accurate program records.
· Resolve discrepancies with service providers.
· Participate in outreach and community events.
· Attend and participate in meetings to explain the program and availability of services.
· Participate in required training programs.
Qualifications
· Bachelor’s degree in social work, gerontology, human services, psychology, sociology, education or a related field and 2 years work experience in a related field.
· Previous experience working with older adults and/or community program coordination.
· Strong organizational and multitasking skills.
· Excellent communication and interpersonal skills, bilingual a plus.
· Ability to work independently and collaboratively with organizational teams.
Special Requirement:
Must possess a valid driver’s license and reliable vehicle.
Work Schedule:
30 hours per week (including evenings and weekends as caseload requires).
Salary: Starting at $29.50 per hour.
