DESCRIPTION OF WORK
Performs to duties and responsibilities as the coordinator for the Maternal Health Home Visiting Program. Coordinates the identification, planning and execution of community based public health initiatives to include home health services and utilization of services performed on the mobile medical unit. Annually reviews and updates the program policies and standing orders. Assists with entering communicable disease event data in the State’s reporting system. The position works under the limited supervision of the Director of Nursing.

Minimum Education and Experience: Associates’ degree in nursing, or related field and one (1) – four (4) years of directly related experience and some supervisory experience in related field or equivalent combination of education and experience.

40% | Post-Partum Newborn Nurse: Receives and picks up referrals and demographic information for post-partum Mothers and newborns. Contacts Mothers to schedule home visits, assemble educational material packets for home visits, provides in-home nurse visits, and assesses the health of the mother and the infant. Educates, intervenes, and refers to resources and services as needed. Informs physicians and pediatricians of visit assessment and findings and enters notes on both the mother and infant into the Electronic Health Record, and makes referrals as needed. Prints copies of notes for billing and faxes to both the mother and Infant’s physicians. Maintains and replenishes supplies for visits. Cleans and sanitizes equipment after each visit and maintains equipment batteries, service, and control checks. Maintains county issued vehicle fuel level, tire, and oil service maintenance, and take for repairs and services as needed. |

10% | Lead Case Management Nurse: Receives and reviews lead records and lead level results from pediatrician’s offices and from the NC Lead website. Makes appropriate calls to obtain contact information for parents/guardian of children, makes calls, ad contacts parents or pediatrician’s offices to assure that follow-up testing is scheduled as appropriate, writes letters, and mails them to those in which phone contact is not obtained. Provides education and counseling to assist with the removal and management of lead source, documents in the NC Lead Program website and the E.H.R. for lead case management. Provides in-home lead case investigations in conjunction with Environmental Health to assist with identifying the lead source, complete forms, obtain consent, and educate/counsel as appropriate for intervention. Makes referrals as appropriate for follow-up testing, state investigations, and additional needs as appropriate. |
10% | Provides back-up support and client management for Communicable Disease/TB Program as required by state agreement addenda. |
5% | Apprises administration of needed infection control policy revisions. Monitors and analyzes accident reports related to infection control. Develops strategies and recommends changes in practices if necessary to reduce or eliminate further accidents of similar nature in collaboration with the County Safety Risk Manager. Serves on the EPI Team. Coordinates annual training required for the agency staff. Coordinates with the Lab Supervisor annually to review equipment and supplies based on surveys conducted by program staff. |
5% | Performs state mandated certification and recertifications, assessments, monitors, audits and serves as a Consultant for the Adult Day Care/Health Facilities within Rockingham County. |

- Valid NC Driver's License.
- Register Nurse upon hire.
- Certified in Basic Life Support (BLS) upon hire.
