Rockaway Home Care is hiring Per Diem Registered Nurses (RNs) and Outpatient Nurses to support our growing patient population throughout Manhattan, The Bronx, Brooklyn, and Staten Island. Key Responsibilities
- Conduct in-home or community-based assessments
- Perform head-to-toe evaluations including medication and diagnosis review
- Complete aide supervisions and provide clinical guidance
- Develop and update personalized Plans of Care (POC)
- Provide orientation to aides based on the POC
- Accurately document findings and maintain compliance with healthcare regulations
- Current New York State RN license
- Ability to travel throughout the 5 boroughs, Nassau, and Suffolk
- Medical Professional Liability insurance
- Comfortable using Gmail, Microsoft Word/Excel, and UAS-NY (for home care roles)
- Bilingual in Spanish/English
- Familiarity with Medicaid, Medicare, or MLTC plans
- Experience in outpatient nursing, home care, or community health
- Proficiency in the UAS-NY platform
- Strong documentation and communication skills
