- Coordinate the intake process for new clients, including patient assessments, insurance verifications, and paperwork completion
- Collaborate with our care team to develop personalized care plans that meet the unique needs of each client
- Communicate effectively with clients, families, and healthcare providers to ensure a smooth transition into our home care services
- Maintain accurate and up-to-date records of client information, care plans, and service delivery
