Responsibilities:
- Travel to client homes across assigned counties
- Complete nursing assessments and reassessments as needed
- Develop, review, and update client care plans
- Monitor client condition and report changes as appropriate
- Provide support, guidance, and supervision to caregivers
- Ensure care is being provided according to the client's needs and care plan
- Communicate with clients, families, caregivers, and office staff
- Maintain accurate and timely documentation
- Help ensure compliance with company policies and home care requirements
- Promote a compassionate, respectful, and professional care environment
- Active RN license
- Prior home care experience preferred
- Willingness to travel to client homes in multiple counties
- Valid driver's license and reliable transportation
- Strong assessment, communication, and documentation skills
- Ability to work independently and manage time effectively
- Professional, compassionate, and reliable
- Must meet all company hiring and onboarding requirements
