STATE CASE MANAGER - Residential Program
Rock Valley Community Programs provides transitional and supportive housing, addiction and mental health treatment services to adults in the Rock County area. We provide these services to homeless male veterans; clients connected to the criminal justice system and outpatient care to anyone needing assistance with addiction and/or mental health treatment.
Position:
The State Case Manager supports the State Program Director by leading case management efforts for residents. The role focuses on developing and coordinating plans to address residents’ risks and needs, ensuring accountability, and supporting successful reintegration into the community.
Shift: Monday-Friday 8am-4pm *flex as needed (typically one late day during the week)
Pay: Starting $20.50/hr, with higher rates based on education and experience.

Key Responsibilities:
- Develop and oversee individualized case plans addressing residents’ core needs (housing, employment, finances, health, education, identification).
- Provide direct support through regular individual and group sessions, ensuring progress, accountability, and successful program completion.
- Monitor resident behavior and compliance, maintaining communication with agents, therapists, and required authorities.
- Coordinate care with internal staff and external partners, including staffing meetings and community resource referrals.
- Maintain accurate documentation, reporting, file audits, and consistent communication to ensure compliance, quality assurance, and program standards.
- Degree and/or experience in social services, counseling, or related field.
- Knowledge of DOC, CBRF requirements, and organizational policies; ability to use community resources effectively.
- Strong interpersonal skills with a trauma-informed, inclusive approach.
- Must meet basic physical demands (sitting, standing, light lifting up to 25 lbs).
- Valid driver’s license required; must complete pre-employment screenings
- Completion of CPR, First Aid, CBRF Fire Safety, and Choking training within 90 days
- Employment is contingent upon passing a state and federal background check.
