Role Overview: The Care Coordinator RN (CC RN) assesses transitional care needs, plans and facilitates patient discharges, and coordinates care across the continuum to ensure efficient and timely progression towards discharge.
Key Responsibilities: Conduct psychosocial and functional assessments, collaborate with patients, families, physicians, and care teams, develop individualized discharge plans, identify and resolve barriers to discharge, facilitate post-acute referrals, and participate in interdisciplinary rounds. Ensure accurate documentation, manage care progression, and support patient education.
Qualifications & Skills: Requires a nursing degree (Associate's or Bachelor's preferred), RN license (single state or multi-state), BLS certification, and at least 1 year of acute care nursing experience. Must possess excellent communication, assessment, organizational, and problem-solving skills, with knowledge of regulations and community resources.
Additional Duties: Support professional development, serve as a mentor, and ensure compliance with policies. The role emphasizes compassionate care, teamwork, and effective resource utilization to promote optimal patient outcomes.
