What are the responsibilities and job description for the Rock County Counseling Center-Outpatient - Human Services Professional III - Therapist position at Rock County?
Through the Rock County Counseling Center, this position provides person-centered, trauma informed therapy and case management services to adults with complex and/or co-occurring behavioral health needs. Daily activities in this position include providing individual and group therapy services, completing intake assessments; completing safety assessments; documenting care provided; attending client staffings and administrative meetings; case management; discharge planning; collaboration with internal and external colleagues. Services are provided in-person in an office setting and in the community. Telehealth services are also provided based on client needs. This position supports and promotes the mission, vision and guiding principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and "Commitment to my Co-Workers."
70% Provide evidenced based therapy and case management services to assigned clients:
5% Will perform other duties as assignedProfessional expertise in the following areas preferred. Training and ongoing professional development will be provided.
- Current Wisconsin LPC, LCSW, or LMFT, or in-training credential will be considered.
- Experience providing Substance Use Disorder treatment preferred.
- Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto
insurance pursuant to the Rock County Administrative Policy and Procedure Policy 5.02 and 5.27.
- Ability to work flexible hours, including some eveningsPhysical Elements
- Ability to stand, walk and sit.
- Ability to exert up to 20 lbs of force occasionally or up to 10 lbs of force frequently

70% Provide evidenced based therapy and case management services to assigned clients:
- Complete co-occurring assessments and diagnoses, including level of care recommendations using the American Society of Addiction Medicine (ASAM) screening criteria.
- Collaborate with clients to develop a comprehensive treatment plan for services, review progress at regular intervals per DHS 75, and make updates to treatment plan and ASAM as indicated.
- Provide evidenced based individual and group therapy to address needs identified in treatment plan.
- Provide referrals to ancillary services as indicated through the biopsychosocial assessment and treatment plan.
- Monitor client drug testing and medication count results.
- Provide case management services to clients, which may include: providing transportation as needed/appropriate, assisting in obtaining housing resources, linking clients with medical treatment and insurance, attending ancillary appointment with clients as appropriate.
- Coordination with the multi-disciplinary treatment team to discuss clients progress, needs and recommendations.
- Participate in discharge planning with the clients prior to the completion of their programming to link them with aftercare treatment services.
- Complete all paperwork in determined time frame.
- Follow DHS 75 requirements for documentation of assessment, treatment plans, reviews, transfers, progress notes, and discharges.
- Participate in supervision, clinical staffings and administrative meetings as assigned.
- Follow all Rock County and Human Services Department policies/procedures.
- Complete documentation, Staff Time Entry and bimonthly time sheets as required by agency policy.

- Knowledge of current evidenced-based best practices related to the assessment and treatment of mental health, substance use and co-occurring disorders
- Knowledge of suicide risk assessment and safety planning.
- Knowledge and application of best practices in cultural competency, person centered care, and Trauma Informed Care.
- Knowledge of Motivational Interviewing principles and skills, to support engagement with clients with complex needs.
- Knowledge of laws, regulations, and practices pertaining to federal and state human service and behavioral health care programs.
- Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.
- Knowledge of psycho-social rehabilitation services
- Ability to establish and maintain effective collaborative relationships within the agency and community.
- Ability to take initiative, work independently, and seek out supervision as needed
- Ability to communicate effectively orally and in writing.
- Ability to understand and carry out verbal and written instructions
- Ability to maintain accurate records and prepare clear and concise reports
