Designated Coordinator Job Summary
The Designated Coordinator is responsible for an assigned case load of individuals as directed by the Program Director with special emphasis on program writing, Vocational Rehabilitation Services, and compliance.
Summary of Responsibilities
- Responsible for monitoring and assuring the quality of delivery of service to clients.
- Participates in the individual planning process with assigned individuals.
- Inspect and maintain files to make sure they meet compliance requirements and contain all required materials (e.g., skill assessment, CSSP Addendum, SMA, IAPP, annuals, quarterlies, psychological test results, releases of information, contact sheets, etc.).
- Responsible for the writing, implementation and review of all program plans, behavior plans, outcome plans and procedures written for consumers. Responsible to insure a well-integrated, coordinated, and effective program for each assigned individual.
- Develop pleasant and respectful working relationships with parents, county social workers, contracted case managers, and other team members to encourage their participation in the person’s program plan.
- Responsible for establishing and maintaining adequate safety procedures and precautions to safeguard the health and well-being of all clients, property, and equipment assigned to their designated area.
- Responsible for developing and implementing realistic training goals and for appropriate and timely communication between staff and others to maximize clients’ chances for achieving their goals.
- To consult with Program Director, Program Manager, other providers, and other community resources in helping the person establish a social/recreational, employment network.
- Perform other duties and responsibilities as assigned by management.
Qualifications:
- Knowledge customarily acquired through post high school education; experience in the field may be substituted for education.
- One-year minimum experience working in and/or supervising a licensed setting.
- Valid Driver’s License and pass vehicle insurance eligibility requirements
- Qualified background check
- Clear demonstration of written and verbal communication skills
- Ability to take initiative; problem solve; make independent decisions; participate actively in meetings.
