Rochester Regional Health is seeking an experienced physician leader to serve as Medical Director for ElderONE, the region’s Program of All-Inclusive Care for the Elderly (PACE). ElderONE provides comprehensive, coordinated care that helps frail older adults safely remain in their homes and communities.
The Medical Director oversees clinical quality, regulatory compliance, and interdisciplinary team performance while also providing direct patient care. This leader partners closely with operational leadership to drive program growth, quality outcomes, and participant satisfaction.
Key Responsibilities:
- Provide clinical leadership for all ElderONE/PACE sites
- Lead and mentor interdisciplinary care teams
- Ensure compliance with CMS and NYSDOH PACE regulations
- Oversee care planning, quality initiatives, and clinical standards
- Contribute to strategic planning, program operations, and performance improvement
Qualifications:
- M.D. or D.O., board certified in Geriatrics, Internal Medicine, or Family Medicine (Geriatrics preferred)
- Eligible for NY medical licensure
- Minimum 5 years of relevant clinical experience; leadership experience strongly preferred
- Strong communication, collaboration, and team leadership skills

PACE provides comprehensive medical and social services to eligible older adults through an interdisciplinary team approach, enabling participants to remain safely at home rather than entering a nursing facility. Services include primary and specialty care, rehabilitation, transportation, social engagement, and 24/7 clinical support.

PAY RANGE: $250,000.00 - $300,000.00
