SUMMARY:
An experienced registered nurse who is responsible for assisting, guiding, coaching, promoting, and executing quality care delivery throughout inpatient and outpatient care “units.” Responsible for rounding and assisting staff nurses in improving quality care and patient experience. Assists in identifying team member educational needs and in teaching team members and patients for the purpose of enhancing a patient-centered care model and the patient's individual plan of care. Assists in the assessment and completion of required core measures. Serves as a role model, staff nurse peer support, and resource for excellence in clinical care and the overall patient experience. Provides superior patient/family customer service to align with the overall expectation of a positive patient experience.
RESPONSIBILITIES:
• Performs initial and ongoing assessment of the patient, utilizing appropriate data (i.e. physical, psychological, educational and risk-screen data appropriate for the chronological/developmental age of the patient). Demonstrates clinical judgment in responding to priority patient needs.
• Communicates with the multidisciplinary team and documents the assessment of the patient appropriately. Develop and document an individualized plan of care, treatment, and services that is appropriate for the patient's needs, strengths, limitations, and goals.
• Demonstrates effective communication with the patient / family and the team while maintaining an appropriate nurse/patient and peer relationship.
• Evaluate, communicate, and document patient responses to care while ensuring the patient receives/understands education/training specific to their needs and abilities. Revise the plan of care based on ongoing evaluations and update documentation appropriately.
• Assists with patient movement and throughput as appropriate.
• Demonstrates flexibility in meeting staffing needs throughout the organization.
• Provides respect for patients' rights by maintaining strict confidentiality of patient information, responding to patients' right to know, and acting as a patient advocate.
REQUIRED QUALIFICATIONS:
- Associate's degree in nursing or diploma from an accredited Nursing School required.
- 3-5 years of acute care experience.
- Current New York State Nursing license required.
PREFERRED QUALIFICATIONS:
- BSN preferred.
PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
PAY RANGE: $41.91 - $55.16
