Job Title: LPN Case Manager – Home Care
Location: Leawood, KS – Hybrid (remote/hybrid)
Schedule: Part-time, 15-20 hours per week, with weekend on-call rotation every 5 weeks
Compensation: Hourly, $50/hr for any case management activities that occur in person and on site, $28/hr for any case management activities that are handled via text or phone

Responsibilities:
Assess clients and their homes to determine care needs
Train, oversee, and support caregivers in providing high-quality care
Update and create individualized care plans
Provide patient and family education on care routines
Serve as a clinical resource for caregivers
Collaborate with office staff and attend occasional in-office meetings/trainings
Qualifications:
LPN license required
Current CPR certification or completion within 90 days of hire
Current TB test or completion within 90 days of hire
Minimum 1 year as a licensed nurse preferred
Strong skills in care planning, patient/family education, and leadership
Licensed driver with a clean driving record and reliable vehicle
Cellphone with internet and texting capabilities; daily access to email
Ability to meet physical and mental demands of the position
Comply with RNS policies, HIPAA, dress code, and weekend/holiday availability
