About the Role
Richard Milburn Academy is seeking a passionate and student-centered Transition Specialist to support students with disabilities as they prepare for life beyond high school.
In this role, you will help students and families navigate post-secondary opportunities, connect with community resources, and develop meaningful plans for education, employment, and independent living. You will collaborate closely with teachers, administrators, families, agency providers, and community partners to ensure students receive the support they need to achieve their goals and successfully transition into adulthood.
This position is ideal for an educator or specialist who is knowledgeable about transition services, enjoys building relationships, and is committed to improving outcomes for students with disabilities.
Qualifications
Required
- Master's Degree from an accredited college or university
- Appropriate credentials from a State Licensing Agency
Preferred
- Three years of high school teaching experience
- Experience with special education transition services and compliance requirements
- Knowledge of state and federal special education regulations
Key Responsibilities
Student Transition Planning
- Develop and support individualized post-secondary transition plans focused on education, employment, and independent living.
- Assist in creating appropriate post-secondary goals, courses of study, and coordinated transition activities.
- Monitor student progress toward transition-related IEP goals and objectives.
- Provide accurate updates regarding students' progress and course completion.
Collaboration & Program Support
- Support teachers in implementing and aligning transition planning across grade levels.
- Collaborate in the development of transition curriculum and instructional supports.
- Partner with the Career and Technical Education (CTE) program to expand opportunities for students.
- Work closely with evaluation staff, teachers, families, and agency representatives to support student success.
Compliance & Coordination
- Coordinate and facilitate all required transition planning meetings within established timelines.
- Maintain current knowledge of state and federal transition requirements.
- Ensure district compliance with transition-related special education requirements, including State Performance Plan Indicators 13 and 14.
- Manage required data collection and reporting within state databases.
- Conduct annual follow-up surveys of special education graduates.
Family & Community Engagement
- Build partnerships with community organizations, colleges, universities, vocational programs, and adult service agencies.
- Connect students and families with resources that support post-secondary success.
- Maintain and share information regarding available community services and opportunities.
- Establish and maintain positive communication with students, families, teachers, and campus leaders.
