245D Designated Coordinator/Designated Manager

Job Overview
We are seeking a highly motivated and organized 245D Designated Coordinator to lead and oversee the implementation of services for individuals with developmental disabilities. This vital role involves coordinating program activities, managing relationships with clients, families, and service providers, and ensuring compliance with state regulations. Duties

• Serve as the primary point of contact for all 245D service coordination, ensuring seamless communication among clients, families, and service providers.
• Develop, implement, and monitor individualized service plans tailored to each person’s needs and goals.
• Manage program operations including budgeting, staffing schedules, and resource allocation to maximize service quality and efficiency.
• Supervise direct support staff and other team members, providing guidance, training, and performance feedback to foster a collaborative work environment.
• Maintain compliance with state regulations by documenting services accurately, completing required reports, and participating in audits or reviews.
• Build strong relationships with community partners and stakeholders to enhance service offerings and support networks for individuals served.
• Lead program development initiatives by identifying opportunities for growth, innovation, and improved client outcomes.

Experience

• Proven management experience in social work, behavioral health, or program management related to developmental disabilities.
• Demonstrated success in developing and implementing programs that support individuals with diverse needs.
• Supervisory experience overseeing teams of direct care or support staff in a social services setting.
• Strong background in working directly with individuals with developmental disabilities to understand their unique needs.
• Administrative experience including budgeting, documentation, compliance tracking, and report writing.
• Excellent relationship management skills to collaborate effectively with families, community agencies, and internal teams.

• Starting pay is $28.00 depending on experience

Pay: $28.00 - $28.50 per hour
