Responsibilities:
· Responsible for facilitating family preservation and support services to eligible families.
· Works in collaboration with the child/adolescent, their family and community partners in a person-centered, solution focused, strength-based approach.
· Assists families in planning, linking, advocacy, coordination, and monitoring of goals, needs and strategies in the community.
Qualifications:
· Bachelor’s Degree in an appropriate mental health discipline.
· CMP-one to three years’ experience depending on degree, providing treatment to children/adolescents.
· Must have reliable means of transportation for home visits and maintain safe driving record.
