What are the responsibilities and job description for the ACT Case Manager position at Riverwood Center?
POSITION SUMMARY: Provides ACT case management services in the community to consumers with severe mental illness and their families. Assists in planning, linking, advocacy, coordination, and monitoring; to assist consumers in gaining access to health services, financial assistance, employment, education, social services and natural supports. Assesses needs, develops, and monitors Person Centered Plans and documents treatment in an accurate and timely manner. Provides video or audio assessments after hours on a rotated schedule between all team members.
EDUCATION: Bachelor’s in social work or in an appropriate mental health discipline.
LICENSURE/CERTIFICATIONS: Licensed by the Michigan Department of Commerce or Professional Regulation as a LBSW, LLMSW, LMSW, TLLP, LLP, LLPC or LPC and must become fully licensed with-in 30 months of date of hire. Must have reliable means of transportation for home visits and maintain safe driving permit.
EXPERIENCE/KNOWLEDGE: Two or more years’ experience providing treatment to individual’s mental illness. Excellent computer and interpersonal skills, punctuality, ability to organize and manage several projects/tasks simultaneously, and work independently.
