Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) specialized in Memory Care to join our dynamic healthcare team. In this vital role, you will provide expert nursing care tailored to individuals living with dementia and other memory-related conditions within a supportive assisted living environment. Responsibilities

• Deliver comprehensive nursing care to residents with memory impairments, including Alzheimer’s disease and other dementias, ensuring their safety and comfort at all times
• Conduct detailed patient assessments, monitor vital signs, and document medical information accurately using electronic health record (EHR) systems such as Epic or Cerner
• Administer medications, including injections, IV infusions, and pain management therapies, following strict protocols and HIPAA regulations
• Assist with activities of daily living (ADLs), including mobility support, catheter care, tube feeding, and hygiene assistance tailored to individual needs
• Collaborate with interdisciplinary teams to develop and implement personalized care plans that address cognitive, physical, and emotional health goals
• Manage acute pain episodes and respond swiftly to behavioral health concerns or emergencies within the memory care unit
• Maintain sterile techniques during procedures such as phlebotomy, IV insertion, suturing, or specimen collection while adhering to infection control standards

Skills

• Proven experience working with individuals with developmental disabilities, dementia care, or in memory care settings
• Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques relevant to senior care
• Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate medical documentation and patient records management
• Skilled in medication administration including injections and IV therapy
• Ability to perform physical examinations, vital sign assessments, and patient assessments efficiently in a fast-paced environment
• Knowledge of infection control practices and aseptic techniques essential for safe patient care
• Excellent communication skills for effective triage, family education, and team collaboration; comfortable working with seniors experiencing cognitive decline or behavioral challenges

Pay: $20.00 - $26.00 per hour
